Job Title: Bilingual Community Health Worker

Location: Austin, TX

Job Type: Full-Time | Exempt

Schedule: Monday–Friday, 8:00 AM–5:00 PM

Pay: $25-30/hourly (compensation range is subject to increase for advanced social work degrees)

About The Role

Addison Group is hiring an experienced and passionate Community Health Worker (Bilingual – Spanish) on behalf of our valued healthcare partner in Austin, Texas. This is a full-time, onsite opportunity serving an innovative care team focused on improving patient outcomes, health equity, and community engagement for Medicare-eligible adults.

Key Responsibilities

As a Community Health Worker, you will be a vital connection between clients, families, and community resources — advocating for older adults, bridging language and cultural gaps, and helping clients navigate complex care systems. Key responsibilities include:

  • Provide community-based support and culturally competent outreach to Spanish-speaking older adults and caregivers.
  • Conduct in-person visits in clinical settings, health education, and follow-ups to assess needs and progress.
  • Help clients understand and access medical appointments, benefits, and social services.
  • Build strong relationships with clients, families, and multidisciplinary care teams.
  • Document client interactions and outcomes in electronic health records.
  • Connect clients to transportation, food, housing, and other essential community support.
  • Act as a trusted advocate to promote engagement, self-sufficiency, and optimal health outcomes.

Requirements

Experience:

  • Bilingual proficiency in Spanish and English (verbal and written).
  • Experience working in medical clinical settings.
  • Experience working in community health, social services, care coordination, or related fields.
  • Strong interpersonal and communication skills with older adults and family caregivers.
  • Comfort engaging clients in their homes and community settings.

Education

  • High School Diploma OR Equivalent (required)
  • Licensed Clinical Social Worker (LCSW) (preferred)
  • Bachelor's in Social Work (preferred)
  • Master's of Social Work (preferred)

Skills

  • Excellent written and verbal communication skills
  • Strong analytical and problem-solving abilities
  • High attention to detail and accuracy
  • Confident in communicating with patients, providers, staff, and leadership
  • Positive, go-getter attitude and willingness to jump in where needed
