Pay Range:$62400-$63400/Yrs

Description
Collaborate with a team of professionals to support children with special medical needs.
Engage in casework services aiming for swift permanency for children and families.
Facilitate training for foster parents in specialized care skills.
Connect children and families with essential community resources.
Maintain comprehensive written case records.
Work closely with medical staff, including conducting home visits.

Key Responsibilities
Promote the organization's mission, vision, and core values.
Provide intensive casework services for children and families.
Train foster parents alongside medical staff.
Conduct outreach to connect families with resources.
Maintain accurate and detailed case records.
Collaborate with assigned RN for case management.

Qualifications
Bachelor's Degree in Social Work or related field required; Master's degree preferred.
Experience in foster care preferred.
Bilingual in Spanish and English preferred.
Strong verbal, writing, and organizational skills.
Ability to work under pressure and meet deadlines.
Proficiency in general computer use.
