Primary Purpose:
Facilitate the implementation of transition and employment services for students with disabilities, assuring that coordinated outcomes are appropriately planned. Provide information and resources to students, parents/guardians, school staff, and agency providers. Support 18+/transition services.
Qualifications:
Education/Certification:
Bachelor's degree from accredited university
Valid Texas teaching certificate in the field of special education
Special Knowledge/Skills:
- Knowledge of federal laws, state requirements, and current regulations related to special education students and programs as well as those specific to transition services
- Knowledge of evidence-based practices related to transition planning
- Ability to function within school and community settings
- Strong computer skills
- Ability to work cooperatively with staff, students, parents, and the community
- Ability to work with team members and model evidence-based practices
- Ability to work with various community and governmental agencies
- Ability to interpret data Excellent communication (written and oral), organizational, and interpersonal skills
- Ability to effectively develop training materials and provide professional development to district staff
- Comprehensive understanding of graduation requirements as outlined by TEA Knowledge and understanding of Person Centered Planning Process
Experience: Three years of teaching experience Experience in implementing transition planning for students with disabilities
Major Responsibilities and Duties: Assessment 1. Recommend age-appropriate transition assessments for use with students.
2. Recommend best practices for staff related to ensuring accurate assessment and utilization of data for IEP planning.
Consultation 3. Provide staff development training to appropriate personnel related to evidence-based practices, documentation, and legal requirements for transition planning.
4. Participate in consultation with staff members and parents regarding student needs as needed.
Program Management 5. Provide feedback to the special education coordinator regarding transition updates and future programming needs.
6. Ensure that IEP documents address required elements of transition as indicated through federal law, state guidelines, and local district policies.
7. Plan transition-related parent workshop offerings.
8. Support 18+/transition services, student enterprises, and community partnerships.
9. Serve as district liaison for outside agencies such as Texas Workforce Solutions, LifePath Systems, and Denton County MHMR.
10. Collaborate with special education coordinator and district CCMR staff on reporting for special education students.
Communication 11. Maintain a positive and effective relationship with supervisors.
12. Effectively communicate with colleagues, students, parents/guardians, community partners, and agency providers.
13. Maintain confidentiality.
14. Observe professional and ethical standards, in accordance with generally accepted community standards and the Professional Educators Code of Ethics.
15. Perform other duties as assigned.
Professional Growth and Development 16. Participate in appropriate professional development meetings to stay current on best practices, changes in law, etc.
17. Participate in Transition and Employment Designee (TED) Community of Practice meetings.
18. Keep current on district course offerings, district and state graduation requirements, endorsement areas, LOTE replacements, and CCMR coding.
